New Jersey Parental Media Consent, Waiver, and Release

This document establishes explicit parental consent for capturing and publishing the media likeness of a minor on digital and physical platforms. In accordance with New Jersey privacy standards, including child care and youth program guidelines under N.J.A.C. 3A:52-6.8, organizations must maintain written authorization before posting images or videos of children online.

I hereby grant Evolutions Power Performance (the "Organization"), its legal representatives, and those acting with its authority, the irrevocable and unrestricted right to take, use, record, publish, and reproduce photographs, video recordings, audio recordings, and digital images of my child.

This includes the right to use these materials in any and all media platforms now known or hereafter developed, specifically including but not limited to:

• Social Media Platforms: Facebook, Instagram, TikTok, X (Twitter), and LinkedIn.
• Video Distribution Channels: YouTube, Onform, and digital streaming platforms.
• Digital Applications & Websites: The Organization's official website and email newsletters.
• Print Materials: Marketing brochures, physical flyers, and press releases.

As parent or guardian, or adult player, I agree to the above with any limits set below.

To protect the privacy and safety of minors under New Jersey guidelines, please choose one option below regarding the use of your child's name alongside their image.

OPTION A: The Organization MAY publish my child's first name alongside their photo/video, but never their last name, address, or other personally identifying details.

OPTION B: The Organization MAY NOT publish my child's name or any identifying text whatsoever. Images and videos must remain entirely anonymous.

• No Compensation: I agree that neither I nor my child will receive any financial compensation, royalties, or residual payments for the creation or use of this media.
• No Right to Inspect: I waive any right to inspect or approve the finished video, photograph, social media caption, or accompanying advertising copy before it is posted or published. Final video edits may be reviewed with the Director but at their discretion.
• Ownership: I acknowledge that all rights, titles, and interests (including copyrights) in the captured media belong exclusively to the Organization.

I hereby release, discharge, hold harmless, and agree to indemnify the Organization, its board of directors, employees, agents, volunteers, and legal representatives from any and all claims, demands, causes of action, damages, or liabilities (including reasonable attorney's fees) arising out of or related to the use of these photographs, videos, or recordings. This includes, without limitation, any claims for invasion of privacy, defamation, right of publicity, or copyright infringement.
